Possible causes:

1. You have a dirty laser lens. By a proper lens cleaner and run it occasionally.

2. You buy cheap media.

3. You are touching the disc surface, getting finger prints and therefore skin oils on the surface.

4. Your PC power supply is being asked to provide more watts than it can. Replace the PSU with a 400 watt or higher unit.
